Care Management

Care management provides comprehensive oversight, coordination, and ongoing support to ensure that all aspects of a client’s recovery and behavioral health care are organized, consistent, and aligned.

This service is designed to help clients and families navigate complex treatment and recovery environments while providing structure, accountability, and continuity of care. Support is individualized and adaptive, with increased engagement during periods of instability, transition, or elevated risk. Care management focuses on stabilization, long-term recovery support, and ensuring that clients receive appropriate and effective care throughout all phases of recovery.

Services may include:

• Coordination with residential, outpatient, and continuing care treatment programs

• Communication and collaboration with therapists, psychiatrists, physicians, and clinical providers

• Family consultation, education, and ongoing support

• Monitoring recovery stability, behavioral patterns, and overall functioning

• Providing structured accountability and relapse prevention support

• Supporting transitions between levels of care, including treatment entry and discharge

• Assisting clients in developing routines and structure that support recovery

• Adjusting recovery strategies based on clinical progress and individual needs

• Supporting clients during transitions from treatment to independent living

• Providing long-term continuing care and recovery maintenance

Care management also includes assisting clients and families in identifying and accessing appropriate treatment and recovery resources based on each client’s clinical needs, personal values, and recovery goals.This may include coordination with:

• Detoxification and residential treatment programs

• Outpatient and intensive outpatient treatment providers

• Individual therapists and psychiatric providers

• Mental health specialists and trauma-informed clinicians

• Peer support and recovery programs, including 12-step and non-12-step approaches

• Faith-based and alternative recovery programs

• Emerging and integrative recovery modalities when clinically appropriate

Sober Coaching

Sober coaching provides individualized, real-world support to help clients establish and maintain stability in early recovery and during high-risk transitional periods. As a Certified Alcohol and Drug Counselor (CADC-II), sober coaching services are informed by clinical training, professional experience, and a comprehensive understanding of addiction, recovery, and behavioral health. This allows for informed, structured support that is responsive to each client’s specific clinical history, recovery needs, and environmental factors.

Sober coaching focuses on helping clients build structure, maintain accountability, and navigate daily life while reinforcing recovery-oriented behaviors. Support is tailored to the individual and may include in-person or remote engagement, depending on client needs and circumstances.

Services may include:

• Providing structured accountability and ongoing recovery support

• Monitoring stability and identifying early warning signs of relapse

• Assisting clients in developing daily routines that support recovery

• Supporting clients during transitions from treatment to independent living

• Accompanying clients to recovery-related appointments, meetings, or clinical services when appropriate

• Assisting clients in managing high-risk environments and situations

• Reinforcing treatment recommendations and recovery planning goals

• Providing objective observation and communication with families or care teams when authorized

• Supporting clients in developing independence, responsibility, and long-term recovery stability

Sober coaching services are coordinated with care management and treatment providers when appropriate to ensure continuity of care and consistent support.

Intervention Services

Intervention services provide structured, professional guidance for families and individuals facing addiction, relapse, or behavioral health crises.

The process begins with confidential consultation and preparation, helping families understand the situation, clarify their concerns, and develop a coordinated plan. Families are guided on how to communicate effectively, establish appropriate boundaries, and approach the individual in a calm, clear, and supportive manner.

Professional facilitation ensures the intervention remains focused, organized, and constructive. The goal is to help the individual recognize the impact of their condition and increase the likelihood of accepting appropriate treatment and support. Intervention services also include coordination with treatment providers, clinical professionals, and facilities to identify appropriate levels of care and facilitate timely admission when treatment is accepted.

Support continues beyond the intervention, assisting families and clients through treatment entry, stabilization, and the transition into ongoing care management and recovery support.
